The application form for a Schengen visa consists of 37 entry fields and 3 declarations. You do not have to fill in the column on the right, it is intended for processing your application.

I declare that I have taken note of and agree to the following: In order to examine my visa application, the data requested in this application form must be collected, a photograph must be taken of me and, where appropriate, my fingerprints must be decreased. All my personal data provided on the visa application form, as well as my fingerprints and the photograph taken of me, will be provided to the competent authorities of the Member States and processed by those authorities for the purpose of deciding on my visa application. This data and the data concerning the decision taken on my application or a decision to cancel, revoke or extend an issued visa are entered into the Visa Information System VIS and stored there for a maximum of five years. During that period, they shall be accessible to the visa authorities and the authorities responsible for carrying out visa checks at the external borders and within the Member States, immigration and asylum authorities in the Member States, so that they can verify that the conditions for legal entry and legal residence on the territory of the Member States, to identify persons who do not or no longer fulfill these conditions, to examine an asylum application and to determine who is responsible for this examination. Under certain conditions, the data is also available to the designated authorities of the Member States and to Europol for the purpose of preventing, detecting and investigating terrorist offenses and other serious crime.

I am aware that I have the right to require a Member State to inform me of the data concerning me stored in the VIS and which Member State has sent this data to the VIS, and that I have the right to request that incorrect data about me will be corrected and that unlawfully processed data about me will be destroyed. At my express request, the authority examining my application will inform me of how I can exercise my right to control my personal data and have it corrected or destroyed, including the related measures under national law. legislation of the Member State concerned.

I declare that all personal data provided by me is correct and complete to the best of my knowledge. I am aware that false statements will in all cases result in my application being rejected or a visa already issued being canceled and furthermore that I may be subject to prosecution under the law of the Member State processing the application. I declare that I will leave the territory of the Member State before the expiry of the validity of any visa issued. I have been informed that having a visa is only one of the requirements for entering the European territory of the Member States. Possession of a visa does not in itself automatically entitle me to compensation if I do not comply with the relevant provisions of Article 5(1) of Regulation (EC) No 562/2006 (Schengen Borders Code) and I am therefore access is denied.
