An explanation of the Schengen visa application form
Any request for one Schengen visa starts by filling out the form'Schengen visa application'. You can fill in this form online and that is highly recommended because of the readability. If it is not possible to fill it in online, you can download the application form for a Schengen visa and fill it in in block capitals.
The application form for a Schengen visa consists of 37 fields and 3 declarations. You do not have to fill in the column on the right, it is intended for processing your application.
Use passport information
It is important that you use the same data for fields 1 -3 (surname, surname and first name) that are in your passport.
Family members of EU, or EEA or CH nationals
Questions marked with an asterisk (*) may not be answered by family members of EU, or EEA or CH nationals (spouse, child or dependent ascendant) exercising their right to free movement. Family members of EU, EEA or CH nationals must prove this relationship with documents and fill in fields 34 and 35.
What are you signing up for?
After filling out the form, sign it by placing your signature. You therefore agree with the 3 statements contained therein. We call them:
I understand that the fee will not be refunded if the visa is refused.
Applicable if applying for a multiple-entry visa (see field 24): I am aware that I must have adequate travel health insurance for my first visit and all subsequent visits to the territory of the Member States.
The third statement is quite lengthy and relates to allowing the issuance of biometric data and the processing of the data provided by the visa applicant. These are stored in the Visa Information System (VIS). You have the right to view this data and to have any incorrect data corrected or removed.
Finally, you declare that all personal data provided by you is correct and complete to the best of your knowledge. And that you are aware that false statements will in all cases result in your application being rejected or a visa already issued being canceled and that furthermore you may be subject to prosecution under the legislation of the Member State processing the application .
You also declare that you will leave the territory of the Member State before the expiry of the validity of any visa issued.
And that you know that possession of a Schengen visa is only one of the requirements for entering the European territory of the member states. Possession of a visa in itself does not automatically entitle you to enter the Schengen area. If you are refused entry because you do not comply with the relevant provisions of Article 5(1) of Regulation (EC) No 562/2006 (Schengen Borders Code), you will not receive any compensation.
It also states that you are aware that upon entry to the European territory of the member states, it will be checked again whether the requirements have been met.
What to do after completing?
You hand in a fully completed and signed online Schengen visa application form on the day of your appointment.
Please note: It is best to complete the form online, print it out and bring it with you. It is strongly recommended that you use the online form instead of filling out a printed form with your handwriting. If you do, write clearly in block capitals.
The Dutch Ministry of Foreign Affairs may contact you by e-mail or telephone for information regarding additional documents or a request for an interview. Make sure that the data is clearly legible and written correctly. It is also recommended that you write the email address in BLOCK CAPITALS.
