Form 'Proof of guarantee and/or private accommodation'
The Proof of sponsorship and/or private accommodation form is completed by the Dutch sponsor guarantor state and/or provides accommodation (accommodation) for the visa applicant.
You can provide a financial guarantee if the visitor has insufficient income. A guarantee is not necessary if the visa applicant can demonstrate that he can spend at least 55 euros per day (per person), for all days that he stays in the Netherlands.
Are you a guarantor for someone who wants to come to the Netherlands for a short stay and are you married? Then you and your partner sign the guarantor statement together. You must leave your signature legalize at the municipality.
You can act as guarantor for several people. You then fill in a separate form 'Proof of sponsorship and/or private accommodation' for each person.
The information you enter on the form is stored in it Visa Information System (the national VIS) as well as the European Visa Information System (VIS) and also used in the Information Supported Decision Making.

How do you fill in the form?
The form is not difficult to fill out. Questions 1 to 11 are about the personal details of the guarantor. The next four questions are about the details of the undersigned's marital/registered partner. The questions that follow are about the statement that accompanies the guarantee and the residence of the visa applicant. Finally, you fill in the details of the foreign national who is subject to a visa requirement.
Remember not to sign your form yet. You must put your signature in the presence of the official of the municipality.
Introduction to the form 'Proof of sponsorship and/or private accommodation'
The form starts with an introduction that you should read carefully before you start filling it in:
You need this form to invite people who require a visa before coming to the Netherlands and to whom you wish to provide accommodation. You also need this form if you want to act as a guarantor for a foreign national who is subject to a visa requirement.
Clearly indicate on the form under 3 'Declaration' what applies in your case: guarantee, accommodation or both. The completed form must be submitted to the municipality for legalization of your signature. NB! In the case of a guarantee, the signature of your partner must also be legalized at the municipality. If you use this form for the arrival of a person who does not require a visa, you do not have to present it to the municipality for legalization of your signature.
You must then send this form to the person you have invited. He must submit the form with his visa application abroad, together with all other documents that can serve to substantiate the visa application.
The data you enter on this form is stored in both the systems of the Dutch authorities responsible for processing short-stay visa applications (the national VIS) and the European Visa Information System (VIS). This data can be shared with the competent authorities in order to optimally facilitate the performance of the public tasks described in the Aliens Act 2000 in the area of inflow, throughflow and outflow. This is done under strict conditions, within the applicable laws and regulations in the field of data protection.
